The Scheme is open to all bona fide water microbiology laboratories in both public and private sectors. Laboratories of all sizes benefit from EQA. The Schemes are of paramount importance to laboratories seeking accreditation for microbiological testing.

* If it is YOUR company policy to refuse payment of invoices which do not show YOUR order number, you MUST provide it here. It is essential that we receive a separate order number for each scheme you wish to join.

In these Schemes, if one laboratory in a non-UK country has volunteered to receive and forward all the samples to the other participants in that country, where there are fewer than five participating laboratories, each will incur partial charges for carriage from Newcastle. Where there are greater than five participating laboratories, no charges for carriage from Newcastle will be incurred.
In the event of the forwarding laboratory withdrawing its services and no substitute being available, or in any event when it is necessary to despatch samples directly to an individual laboratory, full carriage charges will be incurred.

** All non-UK participants will incur carriage charges for sample delivery within this scheme.

Whilst every effort is made to ensure punctual delivery of samples, the Schemes cannot accept responsibility for the time and conditions of transport.

Various aspects of the Scheme may from time to time be subcontracted. When subcontracting occurs it is placed with a competent subcontractor and the Scheme provider will be responsible to the participants for the subcontractor's work.

I have read the conditions of participation for the appropriate Health Protection Agency Water Microbiology External Quality Assessment Scheme(s) and agree to abide by them. I understand that samples issued by the Scheme(s) may contain organisms of ACDP categories 1 and 2 and confirm that my laboratory facilities and staff expertise are adequate for safe handling of these organisms.
